Blepharoplasty (Eyelid Reduction Surgery)

Eyelid reduction surgery can correct excess or droopy eyelid skin that has started to sag over time. It can also remove excess fat, skin, and tissue that have accumulated above and below the eyelids, addressing droopy upper eyelids, bags under the eyes, and sagging eyebrows.

Eyelid surgery can significantly enhance your facial features, particularly the appearance of your eyes. Here are 10 ways blepharoplasty can benefit you:

  • Lift saggy skin in the eyelid area
  • Reduce bags under the eyes
  • Achieve a more youthful appearance
  • Improve the shape of your eyes for a more rested look
  • Correct vision impairment to provide a complete field of vision
  • Smooth crow’s feet and other facial wrinkles
  • Get rid of dark circles under the eyes
  • Eliminate puffiness in the eyes
  • Tighten inelastic skin and muscle
  • Decrease fine lines around the outer lower corners of the eyes
Procedure Cost
Upper Eyelid Blepharoplasty (One) £1900
Upper Eyelid Blepharoplasty (Both) £3500
Lower Eyelid Blepharoplasty (Both) £4000
Combine Upper & Lower Blepharoplasty (All Four Eyelids) £7000

Lower Eyelid Bags Removal

Tired-looking eyes are often caused by puffy or heavy eyelids and saggy under-eye bags. These are common complaints among patients seeking cosmetic surgery. Eyelid surgery, or blepharoplasty, can help restore a youthful and more energetic appearance, making you look refreshed as if you just returned from a holiday.

Causes of Lower Eyelid Bags Lower eyelid bags occur due to the prolapse of orbital fat through weakened tissue known as the septum, which separates the eyelid from the orbit. Additionally, the cheek line drops over the years due to gravity, emphasising the appearance of eyelid bags.

Lower Blepharoplasty Without Scars Some patients may be candidates for a type of lower eyelid surgery called “trans conjunctival blepharoplasty," which involves no external incisions. This superior method involves making incisions within the lower eyelid, leaving no visible scars. This approach requires a high level of skill to avoid changing the shape of the eye. The fatty pockets are repositioned and sculpted from within the lower lid, resulting in a refreshed and youthful appearance, free from under-eye bags that cannot be concealed with cosmetics.

Management Options

  • Mild Cases: Filler injections along the tear trough line can hide mild cases of under-eye bags.
  • Severe Cases: Surgery is preferred for more prominent eyelid bags. The goal is to reduce prominent lid bags and conceal the tear trough line by redistributing fat and performing conservative skin excision. This helps maintain a youthful and tight eyelid appearance.

Cosmetic improvement of the lid bags can be expected by approximately 80%. The cosmetic result should be balanced against the functional outcome to avoid dry eye and exposure of the eye surface.

Risks:

  • Bruising: Bruising may last for about two weeks, with a small amount of bleeding expected on the first day.
  • Double Vision: Rarely, double vision may occur after the surgery, lasting for about two weeks. This happens because the surgery is performed close to the inferior oblique muscle, one of the muscles responsible for eye movement.
  • Scarring: Scarring may be visible for a few months if the surgery is performed from the skin side, but it gradually fades over time.

Preparing for Blepharoplasty

Consultation:

Book an appointment with a surgeon and an ophthalmologist or a specialist in eye surgeries. Most lid surgeries can be performed under local anaesthesia as a day case, allowing you to return home the same day.

Medical History:

The surgeon will ask about your medical history, including past conditions and surgeries, as well as current conditions like diabetes, dry eyes, allergies, glaucoma, circulatory issues, and thyroid problems. Information about medications, vitamins, herbal supplements, alcohol, drug use, and tobacco consumption will also be required.

Setting Expectations:

Have an honest conversation about your motivations and goals for the procedure. The eye surgeon will advise if the procedure can help achieve your desired outcomes.

Before the procedure, you need to complete the following steps:
  • Conduct a physical examination: Your eye surgeon will examine the condition of your eyes and conduct tear production testing. After this, they will measure various parts of your eyelids.
  • Undergo a vision exam: The surgeon will carefully examine your eyes and assess your vision, including your peripheral view. This is essential to determine whether insurance covers the cost of your treatment.
  • Have eyelid photography done: The surgeon will take photos of your eyelids from different angles. These photos will help in planning the surgery and evaluating both immediate and long-term effects, including supporting insurance claims.
Your doctor will also request the following:
  • Possibly adjust or discontinue certain medications that thin the blood or increase bleeding. You should inquire about your medication on the day of consultation.
  • Arrange for transportation to and from the clinic on the day of the outpatient surgery.
  • Refrain from smoking for several weeks prior to the procedure, as it may interfere with the healing process.

What to expect

Before blepharoplasty:

The procedure is typically performed on an outpatient basis. Your eye surgeon will inject a numbing medication into the eyelids, which can be tolerated with or without sedation as needed. Consult with the surgeon if you have concerns about sedation, as most procedures do not require it.

During blepharoplasty:

If the surgery involves both the upper and lower eyelids, the eye surgeon will first work on the upper eyelid. They will make an incision along the fold, remove excess fat, muscle, and skin, then carefully close the incision.

For the lower eyelid, the surgeon will make an incision either below the eyelashes in a natural crease or inside the lower eyelid. They will then redistribute or remove excess fat, muscle, and skin before closing the incision.

In some cases, such as when the upper eyelid droops over the pupil, the surgeon may perform blepharoplasty along with a procedure called ptosis correction to provide additional support to the eyebrow muscles.

After surgery:

Following the advice of your eye surgeon is crucial, although most individuals tend to adhere to the guidelines outlined below:

  • Apply a cold compress immediately after the surgery. Patients can use an eye compress or frozen peas.
  • Remove any compressive eye pads within 2 days after surgery. Steroid ointment can then be applied over the lower lid at least 3 times a day for 1 week.
  • Anti-inflammatory eye drops, which combine antibiotics and steroids, may be prescribed if the surgery is performed on the inner part of your eyelid.
  • Silicone gel may be recommended to minimise scarring.
Before blepharoplasty

Outcomes:

Many individuals are satisfied with the results of the procedure, as it imparts a more youthful appearance and boosts confidence. While for some, the results may last a lifetime, there is a possibility of droopy eyelids recurring.

Bruising and swelling typically diminish within 10 to 14 days. Scars resulting from the incisions may take a few months to fade away. It's important to shield the eyelids from excessive sun exposure to aid in the healing process.
